In this video, the authors demonstrate the surgical techniques used in the case of a patient who presented with an enlarging perineal mass located in the distal rectovaginal septum that was causing discomfort. History was significant for a 4th degree laceration 13 years prior to presentation. The procedure involved a curvilinear incision, sharp and blunt dissection to expose the mass, and dissection aided by electrocautery to excise the mass in its entirety,followed by repair of the anal sphincter and closure of the vaginal mucosa.
