Estimating Risk of Stroke Recurrence

JAMA Neurol; 2016 ePub Feb 29; Arsava, Kim, et al

The Recurrent Risk Estimator (RRE) score provides a valid measure in identifying patients with high and low risk for stroke recurrence. This according to a study of 1,468 consecutive patients with ischemic stroke, including 59 with recurrent ischemic stroke events. Researchers found:

• Cumulative 90-day recurrence rate was 4.2%.

• Mean RRE score was 2.2 in patients with recurrence and 1.0 in patients without recurrence.

• Risk of recurrence increased with a higher RRE score.

• Area under the receiver operating characteristic curve for discrimination was 0.76.

• RRE identified 48.4% of patients in the study as high risk or low risk.

• Sensitivity and specificity were 38% and 93% for identifying low-risk subsets and 41% and 90% for identifying high-risk subsets, respectively.
