A 67-year-old man presents with complaints of fatigue, night sweats, fever, and a 10-lb weight loss in the past 2 months. He describes having to spend a lot of time resting and not being able to complete many daily activities without help or considerable rest between tasks. His abdomen appears distended. He takes a statin to manage hyperlipidemia but is otherwise healthy. Physical examination reveals generalized lymphadenopathy, with several palpable inguinal lymph nodes and hepatosplenomegaly on palpation. Laboratory studies reveal Hb 8 g/dL, LDH 400 U/L, AST 130 U/L, ALT 100 U/L, and AFP 400 U/L. His ECOG performance status score is 3. CT confirms generalized lymphadenopathy with multiple enlarged inguinal lymph nodes.
